Wellness Portal accounts and orders
If you create a Wellness Portal account or place an order, we also collect your name, email address, phone number, password (stored in encrypted form, never visible to us), shipping and billing address, and the details of your orders.
How we use it
We use this information to contact you about your appointment request and to schedule or confirm a visit. We do not sell your information.
Please do not send medical details
The appointment request form on this website is not a secure channel for protected health information. Please do not include medical history, diagnoses, or other sensitive health details in the form. Those conversations belong in the office.
How long we keep it
We keep website enquiries, member account details, and order records only as long as needed to respond to you, provide your care or orders, and meet our business and legal record-keeping obligations. You may ask us to delete your information at any time using the contact details below.

We use trusted service providers to operate this website on our behalf, including our website and customer-relationship platform, a secure database for Wellness Portal accounts and orders, Square for online scheduling and payment processing, and an email delivery service for account and order emails. Card payments are handled by Square; we do not store your full card number.
